Why does a focus on physical well-being matter?

Preventive exams, such as annual wellness exams, screening tests, dental and vision exams help you and your provider prevent illnesses before they cause you symptoms or problems. The frequency for each exam varies by age and gender. These preventive exams are an important opportunity to discuss current health concerns, health history and any questions you may have. This is a time to assess your current health and make lifestyle choices about drinking, smoking, nutrition or physical activity. Annual Wellness Exam

Your Annual Wellness Exam includes age-appropriate labs, weight/height, blood pressure, glucose, cholesterol and age-appropriate screenings.

All Health Plans cover annual wellness exams at 100%, although if a condition is detected, additional tests are needed, or the visit becomes diagnostic, a charge may occur and a bill will be sent.  For teammates not on Atrium Health Plans, contact your insurer for coverage.

LiveWELL-recommended Care Management programs offer uniquely proactive health care education and mentoring for those with specific health conditions and to prevent serious health issues.

Personal Care Management Programs

Atrium Health Greater Charlotte Teammates

Personal Care Management (PCM) is a unique proactive health care program that helps educate and mentor individuals who may show early signs of, or be at risk for, developing serious health issues such as diabetes, uncontrolled blood pressure, uncontrolled asthma, COPD, heart disease, heart failure, hyperlipidemia, metabolic syndrome or multiple co-morbidities. This program is administered by MedCost Benefit Services and is a free benefit available to teammates and their dependents covered under one of the LiveWELL Health Plans. More than 80 percent of participants in the PCM program experience positive health changes that result in fewer days missed from work and fewer health complications. The program provides personal nurse health coaching and mentoring via scheduled phone calls, customized strategies to improve overall health and well-being, care plan designs, educational materials and informative tools tailored to your specific needs. You may receive outreach from this program if your health history suggestions it would be beneficial to you. Or, you can call MedCost directly to find out if you are eligible at 1-800-204-2085. 

Omada Health Program  

Atrium Health Greater Charlotte Teammates

The Omada Health Program is an online group led by a health coach for teammates at risk for diabetes or heart disease. The program is designed to reduce the risk of chronic health conditions while improving overall well-being. Teammates apply to participate in the program; limited number of spots available.

The Omada Health Program is full for 2022 applicants. Stay tuned for details on future opportunities!

  • Weekly online lessons to educate and empower you 
  • A small online community for real-time motivation

Other Resources for Greater Charlotte Benefits Enrolled Teammates 
The following diabetes resources are available to Greater Charlotte teammates who are enrolled in the LiveWELL Health Plan: 

  • Diabetes Care Management (DCM): If you or your family member(s) have been diagnosed with diabetes, you are eligible to participate in Diabetes Care Management (DCM). You have the opportunity to work one-on-one with a MedCost nurse health coach who can help you manage your diabetes and prevent the onset of additional disease or complications. To enroll in DCM, call 1-800-204-2085. 
  • Community Eye Care Diabetes Program: Members with Type 1 or Type 2 diabetes enrolled in a comprehensive Community Eye Care (CEC) plan who undergo a medical exam are eligible for co-pay relief. The member simply submits a form to CEC and is reimbursed for the amount of the co-pay, up to $25. Log in for more information
