Hospital Admission

Annual Wellness Benefit: You may receive a maximum of $50 for teammate and insured spouse screening during a routine preventive care appointment with your doctor. Covered health screening tests include: mammography, colonoscopy, PAP smear, chest X-ray, bone marrow testing, fasting blood glucose test and blood test for triglycerides.

Annual Wellness Benefit: This benefit can help pay the costs for a screening for early signs of disease and lead to earlier intervention. The annual benefit is $100 for completing an eligible health test and is available to each insured person. Some examples are biopsy, chest X-ray, stress test and fasting blood glucose test

Healthcare Power of Attorney and Living Will Educational Opportunity

Advance Directives tell your doctors and your loved ones what you want them to do if something happens to you and you are unable to speak for yourself. With this free educational opportunity you will:

Basic Life Insurance and Accidental Death & Dismemberment (AD&D) 

Atrium Health provides, at no cost to the teammate, financial support with Basic Life Insurance and AD&D to be paid to the teammate's designated beneficiary, if applicable.

  • Benefit Details
    • Basic life insurance is equal to one times the teammate’s annual base salary, up to a maximum benefit of $1,000,000
    • Basic AD&D insurance is equal to one times the teammates annual base salary, up to a maximum benefit of $1,000,0000 
    • Teammates with a minimum of 16 standard hours per week may also purchase supplemental life insurance and/or accidental death and dismemberment coverage for themselves, a spouse or children

Teammates enrolled in the company-provided Basic Life Insurance Plan as of 12/31/2022 will have the following grandfathered coverage: The grandfathered Basic Life Insurance plan is closed to new enrollments/re-enrollments as of 1/1/2023.

  • Eligibility: Full-Time Teammates with at least 30 standard hours per week  
  • Benefit Details:  
    • Basic life insurance is equal to one and one-half (1.5) times the teammate’s annual base salary, up to a maximum benefit of $1,000,000 (effective 1/1/2023). This is an increase in the maximum benefit, up from $500,000

  • Plan varies for monthly paid teammates

    Please note:
    A status change effective 1/1/2023 or later may have an impact on eligibility for the grandfathered Basic Life Insurance Plan. For example, if the teammate experiences a status change which results in standard hours falling below 30, the teammate will lose company-provided Basic Life Insurance coverage.

    Once the teammate’s grandfathered company-provided Basic Life Insurance coverage of one and one-half (1.5 ) times their annual base salary ends, the teammate will not regain eligibility into this grandfathered plan if they experience, at a later date, a status change back to 30 or more standard hours per week. Instead, they would be eligible for the company-provided Basic Life Insurance coverage at one (1) times their annual base salary, up to a maximum benefit of $1,000,000. 

Short-Term Disability

For teammates experiencing an unfortunate illness and/or injury that lasts beyond 14 days, Atrium Health provides access to voluntary short-term disability coverage which pays eligible teammates a benefit equal to 60% of their base salary after satisfying an elimination period of 14 days or 30 days.

    • Full-time and part-time teammates with at least 16 standard hours per pay week
    • Teammates who have reached the first day of the month following 90 days of service in the benefits-eligible status 
  • Benefit Details:
    • Teammates may be eligible to receive payments equal to 60% of their base salary up to $2,500 per week
    • Please see the full STD benefit on the Summary Plan Documents page for more details.  Medical residents do not have a waiting period
    • Plan varies for monthly-paid teammates
Teammates enrolled in the following Short-Term Disability Plans as of 12/31/2022 have grandfathered coverage.   These grandfathered Short-Term Disability Plans are closed to new enrollments/re-enrollments as of 1/1/2023.

      • Teammates may be eligible to receive payments equal to 60% of their base salary up to $2,500 per week

Please note: 
If the teammate experiences a status change which results in standard hours falling below 30, the teammate will lose company-provided Short-Term Disability coverage.  

  • If the teammate’s standard hours are changed to between 16-29, the teammate will be eligible to elect the Voluntary Short-Term Disability Plan and can choose from either a 14-day or a 30-day waiting period option.  
  • If the teammate’s standard hours are changed to 15 or less, the teammate will not be eligible to elect the Voluntary Short-Term Disability Plan.

  • Once the teammate’s company-provided Short Term Disability coverage ends, the teammate will not regain eligibility into this grandfathered plan if the teammate experiences a status change back to 30 or more standard hours per week at a later date.   
  • Voluntary Short-Term Disability Buy-Up
    • Carrier: MetLife  
    • Eligibility: Full-Time Teammates with at least 30 standard hours per week who were also enrolled in the company-provided short-term disability plan as of 12/31/2022  
    • Benefit Details:  
      • Elimination period is 7 days (reduced from 14 days)  
      • Teammates may be eligible to receive payments equal to 60% of their base salary up to $2,500 per week  
      • Please see the full grandfathered STD Buy-Up benefit for more details 

Please note: 
If the teammate experiences a status change which results in standard hours falling below 30, the teammate will lose company-provided and Voluntary Buy Up Short-Term Disability coverage.  

  • If the teammate’s standard hours are changed to between 16-29, the teammate will be eligible to elect the Voluntary Short-Term Disability Plan and can choose from either a 14-day or a 30-day waiting period option.  
  • If the teammate’s standard hours are changed to 15 or less, the teammate will not be eligible to elect the Voluntary Short-Term Disability Plan.

Once the teammate’s company-provided and Voluntary Buy Up Short-Term Disability coverage ends, the teammate will not regain eligibility into this grandfathered plan if the teammate experiences a status change back to 30 or more standard hours per week at a later date.   

  • Voluntary Short-Term Disability for Part-Time Teammates  
    • Carrier: MetLife  
    • Eligibility: Part-Time Teammates with at least 16 standard hours per week  
    • Benefit Details:  

Please note: 
If the teammate experiences a status change which results in standard hours falling below 16, the teammate will lose Voluntary Short-Term Disability for Part-Time teammates coverage. 

  • The teammate will not be eligible to elect the Voluntary Short-Term Disability Plan.

  • Once the teammate’s Voluntary Short-Term Disability for Part-Time teammates coverage ends, the teammate will not regain eligibility into this grandfathered plan if the teammate experiences a status change back to 16 or more standard hours per week at a later date.   

Long-Term Disability

For teammates experiencing an unfortunate illness and/or injury that has lasted beyond 180 days, Atrium Health provides long-term disability coverage which pays eligible teammates a benefit equal to 60% of their base salary up to $10,000 a month.

  • Eligibility:
    • Full-time and part-time teammates with standard work hours of at least 30 hours per pay week
    • Teammates who have reached the first day of the month following 90 days of service in the benefits-eligible status   
    • Teammates who have had at least 180 calendar days of continuous illness or injury that has prevented the teammate from working
    • Plan varies for monthly paid teammates
  • Benefit Details: 
    • Teammates may be eligible to receive payments equal to 60% of their base salary up to $10,000 month

    • Benefits received are considered taxable income

Supplemental Life and Accidental Death and Dismemberment*

Provides additional benefit to you or your loved ones in the event of death or accidental death and dismemberment.

Supplemental Life Coverage

For teammates whose spouse also works for Atrium Health, please note: our plan permits teammates and eligible dependents to only be covered once. For example: if a teammate wants to enroll their child(ren), they will need to determine which parent will carry coverage. Similarly, the teammate's spouse would not be eligible for Supplemental Spouse Life insurance coverage if they hold their own basic life insurance coverage with Atrium Health.  

Teammate
Teammates can elect up to five times their annual base salary up to a maximum of $2 Million
Newly eligible teammates have a guarantee issue of coverage and can increase their coverage by one increment each Annual Enrollment period without completing Evidence of Insurability (EOI)
If teammates choose to increase their benefit by more than one time their annual salary, Evidence of Insurability will be required
Spouse
Teammates can elect coverage for their spouse up to $200,000 in increments of $10,000. Any election over $50,000 will require Evidence of Insurability
Newly eligible teammates have guarantee issue of spouse coverage of $50,000 and can increase or enroll by $10,000, up to $50,000, each annual enrollment period without completing evidence of insurability. If teammates who choose to enroll or increase their spouse benefit by more than $10,000, or if coverage exceeds $50,000, Evidence of Insurability will be required.
Children*
Teammates can elect coverage for their child(ren) up to $20,000 in increments of $2,000
Coverage available to unmarried children from birth to age 26

* Each eligible child is enrolled for the amount elected by the teammate and one premium covers all children.

Accidental Death & Dismemberment

Provides individual life insurance policies that grant a lump sum payment to your beneficiaries upon your death or may help close the financial gap for you and your family due to a chronic or terminal illness.

Dependent Back-up Care

Atrium Health provides a Dependent Back-up Care benefit through LifeCare to connect you with reliable care providers for children and elder care when you suddenly find yourself in need of backup care due to an unexpected circumstance that disrupts regular care plans. All teammates with standard 16 or more standard hours, residents, and weekender positions are eligible for this benefit.  

Registration is FREE:

Provides care to a child or dependent adult when an emergency arises or when regular care plans are disrupted.

  • Eligibility: Teammates with 16 or more standard hours per week — excludes temporary and PRN teammates
  • Enrollment: Teammates may register for this benefit at any time

    • Caregiver vacations
    • Evening and weekend in-home care
    • School holidays or inclement weather
    • Business travel
    • In-home care after minor surgery weather emergencies

    • Eligible teammates may use up to 15 visits, or calendar days, per year!

    • $25 in home use (per caregiver) co-pay

    • $25 per child in center co-pay

    • Up to $60 per day reimbursement for the Friends and Family care model. Registration prior to scheduling care by a friend or family member, and proof of payment to the caregiver is necessary to be eligible for the reimbursement
