Depending on your unique situation, selecting the appropriate level of care is one of the best ways to help you manage costs. LiveWELL Care, formerly known as On-Site Care, locations are located throughout the system to provide increased access to care. Whether you are at work or at home when health symptoms arise, we are available for convenient and reliable care for you, your spouse and your dependents (age 6 months and up) on the LiveWELL Health Plan. We offer on-site visits, visits by phone and video visits.

Time Out for Prevention

Atrium Health encourages all teammates to receive their recommended preventive care. Full-time and part-time teammates with 16 or more standard hours per week, can use up to two hours of paid time off per calendar year for any preventive care and the time may be used in 15-minute increments. Atrium Health Time Out for Prevention may be used for work time missed due to medical, dental, vision, and preventive care. Before using Time Out for Prevention, teammates should discuss with their leader and provide enough notice to ensure the appropriate staffing and time-keeping code is recorded during their time away from work.

Personal Care Management Programs

Personal Care Management (PCM) is a unique proactive health care program that helps educate and mentor individuals who may show early signs of, or be at risk for, developing serious health issues such as diabetes, uncontrolled blood pressure, uncontrolled asthma, COPD, heart disease, heart failure, hyperlipidemia, metabolic syndrome or multiple co-morbidities. This program is administered by MedCost Benefit Services and is a free benefit available to teammates and their dependents covered under one of the LiveWELL Health Plans. More than 80 percent of participants in the PCM program experience positive health changes that result in fewer days missed from work and fewer health complications. The program provides personal nurse health coaching and mentoring via scheduled phone calls, customized strategies to improve overall health and well-being, care plan designs, educational materials and informative tools tailored to your specific needs. You may receive outreach from this program if your health history suggests it would be beneficial to you or you can call MedCost directly to find out if you are eligible, 800-204-2085.

SmartStarts Maternity

SmartStarts will help you get ready to welcome your baby by providing you with resources and information about each stage of your pregnancy. A supplement to your doctor's care, get support from the MedCost SmartStarts nurses who are happy to provide support, resources and answer questions. Enroll in this free program by calling 800-722-2157.

Diabetes Support Programs

Diabetes Care Management (DCM)

If you or your family member(s) have been diagnosed with diabetes, you are eligible to participate in Diabetes Care Management (DCM). You have the opportunity to work one-on-one with a MedCost nurse health coach who can help you manage your diabetes and prevent the onset of additional disease or complications. To enroll in DCM, call 1-800-204-2085.

Participants in the PCM, DCM and SmartStarts Maternity programs may be eligible to earn an incentive for Physical Well-being through the LiveWELL Health Plans.  

CEC Vision Diabetes Program

Members with Type 1 or Type 2 diabetes enrolled in a comprehensive Community Eye Care (CEC) plan who undergo a medical exam are eligible for co-pay relief. The member simply submits a form to CEC and is reimbursed for the amount of the co-pay, up to $25.

One-on-One Rx at CarolinaCARE

The CarolinaCARE mail-order pharmacy website has transitioned to the MyAtriumHealth patient portal. CarolinaCARE will continue to be listed as the preferred pharmacy for teammates and dependents who have selected it for their prescriptions. If teammates do not already have a MyAtriumHealth account, they will need to create one to be able to refill 90-day CarolinaCARE prescriptions within the MyAtriumHealth portal.

The MyAtriumHealth portal allows providers and teammates to manage medical records and prescriptions in one place. A few of the benefits include:

  • Teammates and providers have access to see when prescriptions have been filled and when it’s time for refills.
  • Providers can easily make changes to prescriptions.
  • Teammates can quickly access information 24/7 about their prescriptions with a new QR code label.
  • Enhanced security measures to protect teammates' information.
  • Efficiency in the refills request process by using the “Refill Request” button, which will only be available to patients using an Atrium Health Pharmacy, including CarolinaCARE.

Many people with diabetes are on multiple prescription medications, which can be expensive. One-on-One Rx is a service designed to provide free medication reviews for Atrium Health teammates and their dependents. One-on-One Rx is a team of Atrium Health pharmacy professionals who are experts in the LiveWELL Pharmacy Plan and CarolinaCARE Pharmacy. Our pharmacists strive to maintain and enhance therapeutic outcomes while saving our teammates money on their prescriptions.

Medical Nutrition Therapy

Atrium Health has Registered Dietitians/Nutritionists (RDN) available for outpatient visits to help you reduce your health risks. Health professionals agree that nutrition education is one of the first treatments that individuals should receive to prevent or improve conditions such as diabetes, heart disease and hypertension.

Teammates and family members who are enrolled in one of the LiveWELL Health Plans may receive free visits with a Registered Dietitian Nutritionist (RDN). Appointments are available to participants who have an appropriate physician’s referral for Medical Nutrition Therapy (MNT). Up to six visits may be covered (no charge) in a benefit year to provide appropriate guidance and education for diet-related conditions or risk factors.

How to Get a Referral 

  • Contact the Dietitians’ office of your choice for guidance in securing the referral; or
  • Talk to your physician about a referral

Note that not all Nutrition and Diabetes services are covered at 100%. Please confirm coverage with your caregiver and MedCost prior to service.
