Below is an overview of what to expect regarding when your benefits end due to separation of employment or becoming benefits ineligible. 

Beginning June 3: Log in to totalrewards.org to access personalized benefits and rewards information and enroll in/update your benefits elections.

Health, Dental and Vision Benefits

  • Health, dental and vision coverage ends at the end of the pay period in which you are no longer benefits eligible.  
  • Teammates enrolled in one of the health plans and/or in the dental benefit will have the option to elect COBRA for each covered person. A COBRA notice will be mailed to your home address.
    • If you elect COBRA, benefits will begin the day following the last day of coverage.   
  • Teammates enrolled in the vision benefit will have the option to continue the benefit. Learn about the CEC Vision Portability Plan and contact CEC Vision at 888-254-4290.   

Savings and Spending Accounts  

  • Health Savings Account (HSA): contributions stop with the last regular paycheck. Your account then becomes a retail account with Bank of America. You may continue to use this account for eligible expenses; some fees may apply. 
  • Health Care Flexible Spending Account (FSA): coverage ends on the last date of employment or the date you are no longer benefits eligible. A COBRA option is available through the current year-end. Teammates may file claims up to 120 days after the benefits ineligible date for any expenses incurred through the end of coverage. 
  • LiveWELL Incentive Account: coverage ends on the last date of employment or the date you are no longer benefits eligible. Teammates may file claims up to 120 days after the benefits ineligible date for any expenses incurred through the end of coverage. 
  • Limited Purpose Flexible Spending Account: coverage ends on the last date of employment or the date you are no longer benefits eligible. Teammates may file claims up to 120 days after the benefits ineligible date for any expenses incurred through the end of coverage. 
  • Dependent Care Flexible Spending Account: coverage ends on the last date of employment or the date you are no longer benefits eligible. Teammates may file claims up to 2 ½ months after the end of the plan year for expenses incurred before the end of coverage.

Life Insurance Plans 

  • Coverage ends on the last date of employment or the date you become benefits ineligible. 
  • You may apply to convert your life insurance policy from a group plan to individual coverage. Contact MetLife to request forms at 877-275-6387. 

Short-Term and Long-Term Disability Plans 

  • STD and LTD coverage ends on the date that you become benefits ineligible. Teammates enrolled in Long Term Disability have the option to convert to an individual policy. Contact MetLife to request the forms at 800-929-1492, prompt 5. 

Voluntary Benefits 

  • Coverage ends on the date that you become benefits ineligible. Information regarding how to continue coverage will be mailed from the voluntary benefits vendors. 

Retirement Savings Plan 

  • Contributions to the 401(k) and ADVANTAGE Retirement Plans will stop with the last regular paycheck.  
  • Within 60 days of the last regular paycheck, information regarding distribution options will be mailed from Empower Retirement Services to the teammate’s home address.

Paid Time Off (PTO)

  •  PTO accrual ends on the last day of the pay period in which the end of employment date or date no longer benefits eligible falls. 
  • Earned PTO hours are paid within one to two paychecks after the date of separation or date no longer benefits eligible. The PTO payout is per HR Policy 3.05 Paid Time Off Benefit
    • If separation date is in the first 90 days of employment, teammate receives no earned PTO hours 
    • If separation date is after completion of the pay period in which the 90th day falls, up to the second year of service with Atrium Health, teammate receives 50% of earned PTO hours 
    • If separation date is any time after the second year of service with Atrium Health, teammate receives all earned PTO hours.  

Premiums

  • Upon termination or your move to benefits ineligible, premiums will be deducted through the end of the pay period in which your coverage was in effect.
