W-2 Information

W-2 Tax Documents Now Available in Workday

 To access your W-2 form, follow these steps:

  1. Go to Workday and type “tax documents” in the search bar at the top.
  2. Select My Tax Documents.
  3. View or print your W-2 form.

Teammates are responsible for providing current contact information in Workday. If your address was not updated and you do not receive your W-2 in the mail by Feb. 15 or want access to it at any time, please open a Workday Help case.

IRS Form 1095-C

Note: 1095-C documents are available from totalrewards.org.

The 1095-C form provides information about the health insurance coverage Atrium Health offers to you as a teammate and may be used for your tax preparation. In March, this form will also be distributed by ADP in the same manner as your W-2 form. 

Former Teammates

To access your Atrium Health W-2 and 1095-C document, visit  https://my.adp.com. 

  • If you have previously registered with ADP, log in using your UserID assigned by ADP at the time you previously registered and password previously created 
  • If you have not previously registered with ADP: 
    • Click Create Account to get started 
    • On the next screen, click I have a registration code 
      • Greater Charlotte teammates: enter Registration Code CHZ1-AH
      • Floyd teammates, enter Registration Code FLOYDHLTH-FHM1
      • Navicent teammates, enter Registration Code NAVICENT-NCV1 
      • Wake Forest Baptist teammates, enter Registration Code WFB1-AtriumWH
    • Follow the instructions on screen to complete your registration

Update your state and federal tax income tax withholding in Workday.

Tip: Review your tax withholding information any time your personal or financial situation changes.
