Below is an overview of what to expect regarding when your benefits end due to separation of employment or becoming benefits ineligible. 

Beginning June 3: Log in to totalrewards.org to access personalized benefits and rewards information and enroll in/update your benefits elections.

Health, Dental and Vision Benefits

  • Health, dental and vision coverage ends at the end of the pay period in which you are no longer benefits eligible.  
  • Health Insurance: Teammates enrolled in one of the health plans will have the option to elect COBRA for each covered person. Information regarding continuing coverage, and the cost, will be mailed to your home address.  
    • If you elect COBRA, benefits will begin the day following the last day of coverage 
    • Alternatively, you may also visit www.healthcare.gov or call 800-318-2596 for information on the Healthcare Marketplace insurance plans.  
  • Dental Insurance: Teammates enrolled in the dental benefit will have the option to elect COBRA for each covered person. Information regarding continuing coverage, and the cost, will be mailed to your home address.  
    • If you elect COBRA, benefits will begin the day following the last day of coverage.  
  • Vision Benefit: Teammates enrolled in the vision benefit will have the option to continue the benefit. Learn about the CEC Vision Portability Plan and contact CEC Vision at 888-254-4290.    

Savings and Spending Accounts 

  • Health Savings Account (HSA): contributions stop with the last regular paycheck. Your account will transition to a retail account with Bank of America. You may continue to use this account for eligible expenses; some fees may apply. 
  • Health Care Flexible Spending Account (FSA): coverage ends on the last date of employment or the date you are no longer benefits eligible. A COBRA option is available through the current year-end. Teammates may file claims up to 120 days after the benefits ineligible date for any expenses incurred through the end of coverage. 
  • LiveWELL Incentive Account: coverage ends on the last date of employment or the date you are no longer benefits eligible. Teammates may file claims up to 120 days after the benefits ineligible date for any expenses incurred through the end of coverage. 
  • Limited Purpose Flexible Spending Account: coverage ends on the last date of employment or the date you are no longer benefits eligible. Teammates may file claims up to 120 days after the benefits ineligible date for any expenses incurred through the end of coverage. 
  • Dependent Care Flexible Spending Account: coverage ends on the last date of employment or the date you are no longer benefits eligible. Teammates may file claims up to 2 ½ months after the end of the plan year for expenses incurred before the end of coverage.

Life Insurance Plans

  • Coverage ends on the last date of employment or the date you become benefits ineligible.  
  • You may apply to convert your life insurance policy from a group plan to individual coverage. Information regarding how to continue coverage will be mailed to the teammate’s home address from New York Life, and elections must be made within 30 days from your last day of active employment. Contact New York Life to request forms at 800-362-4462.  

Short-Term and Long-Term Disability Plans 

  • Your coverage in the short-term disability benefit, if elected with The Hartford, and long-term disability benefit, which is company-paid with New York Life, will end on your last day of active work or day you are no longer in a benefits-eligible status.  

Voluntary Benefits 

  • Coverage ends on the date that you become benefits ineligible. Information regarding how to continue coverage will be mailed from the voluntary benefits vendors.  

Retirement Savings Plan 

  • Contributions to the 403(b), 457 Retirement Savings Plans will stop with the last regular paycheck.  
  • If you are a participant in the Atrium Health Navicent 403(b) or 457 Retirement Savings Plans, contact.Corebridge Financial directly about your options for your balance. You may call one of the local advisors at 478-633-2609 or the Corebridge Financial Customer Service Center at 800-448-2542. 
    • Please note:  In order to close out your Corebridge Financial account, your end of employment date must be entered into the Atrium Health Navicent HR system and transmitted to Corebridge Financial through the normal course. The process may take up to 30 days.  
  • If you are vested in the Atrium Health Navicent Pension Plan, you should have received a final pension statement. If you have not received a statement, or if you need an additional copy, please contact the Atrium Health Navicent Pension Center at 650-351-8217. 
    • Please note:  Benefit accruals ended December 31, 2013. 

Paid Time Off (PTO) 

  • PTO accrual ends on the last week worked. 
  • Eligible earned PTO hours will be paid per the PTO policy: 
    • If separation date is in the first six months of employment, teammate receives no earned PTO hours 
    • If separation date is after completion of the pay period in which the sixth month falls, teammate receives 100% of earned PTO hours.  

Extended Illness Bank (EIB) 

  • Extended Illness Bank is not paid out at the time of a teammate’s separation from the organization. 

Premiums 

  • Upon termination or your move to benefits ineligible, premiums will be deducted through the end of the pay period in which your coverage was in effect.
