What is the best way to get started in the LiveWELL Programs and Incentives in 2021?
All teammates, start by taking the Personal Health Survey and receive a personalized health summary to learn more about your health. Then, you can explore which LiveWELL opportunities will best suit your needs and positively impact your overall well-being. This is the first step in the journey to being your best you!
Members of one of the LiveWELL Health Plans can begin earning Incentives after completing the Personal Health Survey on the Total Health Portal. Please note, the Personal Health Survey is not an incentive activity, but is required to complete to receive your incentive rewards.
How do I get to the Total Health Portal to complete my Personal Health Survey?
What is the maximum amount of LiveWELL Incentives that can be earned in 2021?
For teammates who are members of one of the LiveWELL Health Plans, LiveWELL Incentives reward up to $675 for Teammate-Only coverage - $225 each for activities that promote Physical, Financial and Personal Well-Being.
For teammates with Family coverage, up to $1,350 in Incentives can be earned - $450 each for activities that promote Physical, Financial and Personal Well-Being.
When are the Incentives earned in 2021 paid?
Incentives earned in 2021 will be paid in 2021. There are three opportunities for payment:

Incentive dollars are deposited in the Health Savings Account (HSA) for teammates on the Health Savings Plan, and in the LiveWELL Incentive Account for teammates on the Co-Pay Plan.
If you are enrolled in the Co-Pay Plan, the LiveWELL Incentive Account is for Atrium Health contributions only and can be used for qualified medical expenses, including over-the-counter medications.
Note: Teammates on one of the LiveWELL Health Plans and in positions with annual base salaries less than $30,000 will receive $200 in their Health Savings Account if on the Health Savings Plan or LiveWELL Incentive Account if on the Co-Pay Plan.
Can I contribute to the LiveWELL Incentive Account?
No, only LiveWELL Incentives you earned can be contributed by Atrium Health to the account.
What happens to any remaining funds in my accounts at the end of the year?
Different account types have different rules based on IRS guidelines.
What happens to the unused dollars earned in the LiveWELL Incentive Account when I terminate employment or retire from Atrium Health?
When you terminate employment or retire from Atrium Health your unused funds will be forfeited.
Is there an additional Incentive earned for completing all three Incentive dimensions by the first deadline?
There is no additional incentive for completing all three dimensions early; however, it does mean contributions to your HSA (if enrolled in the Health Savings Plan) or to your LiveWELL Incentive Account (if enrolled in the Co-Pay Plan) will take place in April and you will have access to those funds for qualified medical expenses early in the year.
If I have a change in coverage from Individual to Family or from Family to Individual in the middle of the year, will the incentives I’ve earned and received payment for be adjusted accordingly?
Incentives earned and paid prior to a change in coverage will not be adjusted up or down for the new coverage. Any incentives earned after the coverage change will be paid at the current coverage level.
If I had my Annual Wellness Exam after the close of 2020 Incentives and won’t have one again until after the October 18, 2021 deadline for the 2021 Incentives, will I get credit for my Annual Wellness Exam?
Yes, Annual Wellness Exams, as well as participation in an Atrium Health Serves event that were completed on or after October 1, 2020 will be accepted for 2021 Incentive participation. Teammates will simply record the date of their Annual Wellness Exam and/or the date they participated in an Atrium Health Serves event in the Total Health Portal. The date the Total Health Portal is updated with the completed exam or event will determine the date the Incentive will be paid based on the 2021 deadlines/contributions table above. For example, if the teammate’s Annual Wellness Exam was completed on November 25, 2020 and they record that date in the Total Health Portal before March 8, their Incentive will be paid on April 16.
How do I get to the Total Health Portal to log my Annual Wellness Exam or Atrium Health Serves event?
Will there be seed money and a match from Atrium Health to the teammate contributions made in 2021?
There will not be a one-time contribution at the beginning of the year, what was known as seed money, nor will there be a match to teammate contributions in 2021. The LiveWELL Health Plans for 2021 provide several ways to save in healthcare costs including an added Value Network with a lower coinsurance responsibility for the teammates, considerably lower deductibles, lower premiums for families on the Health Savings Plan and a new Co-Pay Plan.
Note: Teammates who make less than $30,000 annually and are members of one of the LiveWELL Health Plans will receive an additional $200 in their Health Savings Account or LiveWELL Incentive Account.
Will there be opportunities to earn the Healthy Weight Reward and Family Health Reward in 2021?
There will not be separate incentives for Healthy Weight or Family Health in 2021. The LiveWELL Incentives have a simpler design with three opportunities to earn. Options in 2021 focus on physical, financial and personal dimensions and provide choices to help teammates continue their focus on healthy weight and engage their family in many healthy activities. Teammates who are members of one of the LiveWELL Health Plans, can earn up to $675 for Teammate-Only coverage, and up to $1,350 for Family coverage.

Can a teammate who is not on a LiveWELL Health Plan earn Incentives?
The Incentives are funds paid out to teammates on the LiveWELL Health Plans. However, all teammates are encouraged to participate in the many programs and education available.
